1. Provides safe therapeutic care in a holistic and systemic way to include the assessment and overall care management of the pre-operative patient and family under the supervision of the Anesthesiologist /PAS Medical Director.

a. Performs physical assessment on identified patients to assess preoperative care needs.

b. Obtains detailed nursing history when applicable and documents that information accurately in Soarian.

c. Consults with the patient primary care/attending or specialist regarding patient assessment findings.

d. Identifies health problems of the patient and communicates this to appropriate care providers - i.e. PCP, Anesthesiologist, Surgeon. Arranges appropriate referrals as needed.

e. Communicates directly with an Anesthesiologist any concerns about the patient's risk for anesthesia.

f. Orders diagnostic testing and consults as required by plan of care, reviews test results pertinent to the patients risk for anesthesia.

g. Demonstrates the ability to review and evaluate the plan of care for effectiveness of desired outcomes.

h. Assesses patient learning needs and provides education related to anesthesia evaluation.

i. Documents or dictates all patient care activities as per WDH documentation standards.

j. Initiates appropriate emergency treatment in life threatening or unanticipated situations in order to stabilize the patient.

2. Assesses the pre-operative patient's risk of anesthesia. Requires access to patient information: inpatient, outpatient and office records for purposes of documenting risk of anesthesia.

. Gathers appropriate information in order to identify the anesthesia risk for patients.

a. Identifies potential risks for anesthesia and collaborates as appropriate with surgical, primary care and specialty offices with regard to the patient's status and optimization prior to anesthesia.

b. Elicits physical and mental health status, psychosocial history, medication history and records findings in H&P when required.

c. Reviews findings on the pre-operative screening tool completed by RN's., reviews diagnostic tests and medical clearance reports to aid in the assigning the risk of anesthesia.

3. Communicates effectively and works cooperatively with others. Has respect for and understanding of other clinical disciplines. Uses an integrated approach to patient outcomes.

. Serves as a change agent by implementing practice initiatives.

a. Coordinates patient care activities to maximize patient outcomes.

b. Acts as a primary resource to the Pre-admissions team regarding surgical preparation.

c. Collaborates with nursing staff, clinicians, physicians, Hospitalists, Surgeons and Anesthesiologist to achieve optimal patient outcomes.

4. Utilizes standards, guidelines, and protocols for care delivery. Incorporates data and information to continuously improve care and practice to enhance outcomes.

. Is a leader in the quality improvement process. Recognizes trends in patient populations, explores alternatives to care and assist in the revision of standards, guidelines and or protocols.

a. Analyzes practices that contribute to variance, draws conclusions from aggregate data, identifies resources to assist in variance analysis and assist with drawing conclusions from aggregate data.

b. Assists in the development and revision of standards for per-operative surgical patients incorporating an interdisciplinary approach and utilizing best practices and research findings.

c. Develops/revises patient education programs and evaluates effectiveness of learning.

d. Reviews current nursing research literature and demonstrates application to clinical practice.

e. Focuses on patient satisfaction when interacting with patients and their families. Actively participates in service excellence activities and identifies opportunities for improvement.

5. Ensures improvements in practice settings by assuming responsibility for self-development and continuing education. Provides direction and guidance to other staff when applicable.

. Recognizes behaviors that impact the team function and implements strategies to optimize teamwork: serves as a mentor for team members.

Qualifications

Experience Minimum Required

• Three to five years of progressive clinical nursing experience with primary care management of patient population.

Education Minimum Required

• Bachelors in Nursing ,and Masters in Advanced Clinical Practice or graduate of post masters certificate program for nurse practitioner.

Special Skills Minimum Required

• Verbal and communication skills, presentation skills, computer skill

Licensure and/or Certifications Required

• Adult or Family NP Certificate RN.

• Certification for advanced practice Licensed or eligible for licensing in the State of New Hampshire.

• AHA or ARC BLS, APRN, Registered Nursing License
