The Behavioral Health RN is a core member of the collaborative care team, which includes the patient's medical provider, counselor, and psychiatric provider. The Behavioral Health RN is responsible for supporting and coordinating behavioral health services that are integrated within the medical department at Finger Lakes Community Health in accordance with the New York State Department of Health Article 28 regulations for Diagnostic and Treatment Center. Main Responsibilities: Support the mental and medical health care of patients on an assigned patient caseload. Closely coordinate care with the patient's medical provider and all behavioral health providers. Screen and assess patients for common mental health and substance abuse disorders. Facilitate patient engagement and follow up care. Provide patient education about common mental health and substance abuse disorders and the available treatment options. Systematically track treatment response and monitor patient s(in person or by telephone) for changes in clinical symptoms, treatment side effects, and or medication complications. Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment. Provide brief behavioral health interventions using evidence-based techniques such as behavioral activation, problem solving treatment, motivational interviewing, or other treatments as appropriate. Provide or facilitate in-health center or outside referrals to evidence-based treatments as clinically indicated after a team-based decision outside of the organization (social services, mental health inpatient, substance abuse detox and rehabilitation, OMH/OASAS treatment providers). Participate in regularly scheduled caseload consultation with the behavioral health team and communicate resulting treatment recommendations to the patients' medical provider. Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan. Including, those patients that are high risk either due to non-compliance or self-harm. Case reviews may be conducted by phone, video, and in person. Track patient follow up and clinical outcomes using a registry. Document in person and telephone encounters in the registry and use the system to identify and re-engage patients. Document patient progress and treatment recommendations in eCW and other required systems to be shared with medical providers, psychiatry, and behavioral health team, or all integrated services. Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health and substance abuse health care. Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload. Be the coordinator for the medication assisted therapy (MAT) program and work closely with medication prescriber and counselor, continuously. Obtain and observe (when required) urine drug screens for MAT when needed at appointments with medical provider. Effectively engages patients in a therapeutic relationship, when appropriate. Education and Qualifications: A current, unrestricted NYS Registered Nurse License required CPR certification required (can be provided) 1-3 Year of psychiatric or behavioral health experience The ability to work flexible hours -- some evenings are required The ability to communicate fluently in both Spanish and English is preferred Physical Requirements: Must be able to hear and communicate with clients and staff via phone, web media, and in person.
