Voyage Healthcare is seeking a RN - UCare for Seniors Care Coordinator to join our team. This position is responsible for delivery of care management in the clinic setting delegated in subcontracts with health plans which are contracted with CMS and DHS. This remote position is part-time, 16 hours/week (Thursday s and Friday s) between the hours of 8:00 AM - 5:00 PM. Responsibilities: Provides comprehensive assessment or screening of each member s bio-psycho-social and environmental safety needs via telephone and utilizes assessment tools within 30 days of enrollment Completes patient assessments via assessment tools within 30 days of enrollment Conducts follow-up visits via telephone screenings on a timely basis Performs review of medical records and treatment plans when available Develops Plan of Care based on assessment maximizing the level of self-determination and member choice of services, service providers and living arrangements to meet identified needs Incorporates interdisciplinary/holistic and preventive focus which includes disease management, advance directive planning and vaccination updates Provides culturally appropriate care Communicates care needs and plan of care with member and/or member representative, primary physicians, and interdisciplinary team members through written and/or verbal communication Utilizes holistic assessment approach with input from team as needed to address social and environmental needs Manages member care needs across settings of care Contacts appropriate personnel in hospital, nursing facility, foster care, or assisted living settings to share and obtain necessary information that will facilitate discharge to the member appropriate level of care Communicates with service providers and members while assisting with arrangement and authorization of services and issue denials of inappropriate services in a timely manner
