Position Summary
The Social Worker delivers comprehensive social work services to patients and their families. This role involves gathering, analyzing, and evaluating data related to medical psycho-social programs; developing and executing treatment plans; and collaborating with the home care interdisciplinary team to ensure patients are safely maintained in the community or transitioned to an appropriate facility.
Essential Functions and Responsibilities:
Adhere to Performance Standards, Policies, and Behaviors. Conduct interviews with patients and/or their families/significant others to assess ongoing care needs. Complete comprehensive social work assessments for all patients referred or needing social services, in line with departmental high-risk criteria. Ensure continuity of care by providing psychosocial information for patients transferring to other units. Offer supportive counseling, crisis intervention, and grief counseling to patients and their families/significant others as needed. Assist patients in adjusting to treatment, addressing issues such as depression, anxiety, denial, anger, and noncompliance with medical regimens. Develop and implement social work treatment and continuing care plans involving the patient, family/significant others, and the interdisciplinary team, documenting in the patient's medical record according to departmental policy. Provide information, referrals, and advocacy to community agencies. Act as an advocate and liaison for patients with community agencies. Coordinate interdisciplinary case conferences as necessary. Supervise caseworkers when designated. Provide direct or task supervision to social work assistants and/or social work students when assigned. Lead education and support groups for patients and/or families/significant others as appropriate. Identify, evaluate, and design safety plans for victims of crimes/abuse, such as child abuse, elder abuse, domestic violence, rape, and sexual assault. Maintain and complete case activity statistics as outlined by departmental policy. Monitor performance by reviewing medical records for accurate documentation. Attend individual, unit, departmental, and intra/inter-hospital meetings as required by the supervisor and/or department director. Participate in presentations, training, and in-services as needed. Engage in the department's cross-training program. Respond to and participate in external and internal disasters when assigned by the supervisor. Perform additional duties as required.
Education & Experience Requirements:
- Master of Social Work (MSW) degree from a program accredited by the Council on Social Work Education.
- Licensed Social Worker (LMSW/LCSW) certification approved by the State Education Department of New York.
- Two years of experience in an acute care hospital setting or a related environment.
- NYS SCREEN Certification must be obtained within the first three months of employment.
