Social Work Specialist

Montefiore Medical Center is seeking a skilled Social Worker to join our team in Bronx, New York.
About the RoleAs a professional member of an interdisciplinary team, the Social Worker will provide a range of direct and indirect services for patients/families, including identifying psychosocial problems and their resolutions. The Social Worker will collaborate with the patient/family/team to effect safe and timely discharge/treatment plans.
RequirementsThe ideal candidate will have a Master's degree in Social Work from a program registered by the NYS Education or determined to be substantially equivalent by the commissioner's regulations. A LMSW or LCSW is required.
