HarmonyCares is a leading home-based primary care practice dedicated to providing coordinated and high-quality healthcare in the home. Our mission is to bring personalized healthcare to patients who have difficulty accessing care. We are looking for a Community Outreach Coordinator to join our team and help enroll eligible patients into the HarmonyCares program.
Responsibilities:
- Lead community outreach activities and build relationships with healthcare facilities and physician offices
- Conduct thorough research on community provider practices and patient healthcare needs
- Educate community providers on the HarmonyCares program and engage patients into the program
- Collect, analyze, and report patient outcomes to community physicians
- Conduct telephonic outreach to enroll patients in the HarmonyCares program
- Be the face of HarmonyCares in the community and collaborate with physician leaders
- Meet or exceed established monthly engagement targets
Qualifications:
- College degree or 5 years of progressive career growth
- 3+ years of related experience in sales, account management, community outreach, or patient engagement in a medical/healthcare industry
- Proficiency in Microsoft Excel, Word, and PowerPoint
- Dynamic and engaging personality with excellent communication skills
- Detail-oriented, compassionate, and a continuous learner
Preferred Skills:
- Bilingual in Spanish, Mandarin, or other languages
- Comfort with CRM and EMR tools
Pay Transparency
Individual compensation packages are based on various factors unique to each candidate
