The Targeted Case Manager will provide quality mental health case management services to clients and their families. Case managers work closely with the children and families in assessing the client's strengths and needs, providing clients linkage with appropriate community resources; providing advocacy at school and court system, and empowering families to become self-sufficient. This position uses the Wraparound model of care, including assessment, developing and reviewing plans, and determining discharge criteria.

Tykes & Teens is a CE-Broker supported organization and we provide over 12 hours of continued education
per year in addition to access to other professional growth opportunities.

The experience you will bring:

  • Bachelor's degree in Human Services or a related field from an accredited university or college
  • A minimum of one year of full-time or equivalent experience working with children with serious emotional disturbances (SED)
  • Completion of Florida Certification Board (FCB) approved mental health-targeted case management training within 90 days of initially providing Medicaid services.
  • Knowledge of currently available resources for children with SED
  • Knowledge of and compliance with state and federal statutes, rules, and policies that affect children.
  • Experience in delivering high-quality, client-centered care.
  • Ability to demonstrate both good communication and listening skills.
The pay range is dependent upon qualifications and experience.
