Job Summary

We are seeking a highly skilled and compassionate Clinical Transplant Coordinator to join our Renal and Pancreas Transplant team at Inova Health System.

The successful candidate will be responsible for coordinating the care of patients undergoing renal and pancreas transplantation, including pre-transplant evaluation, post-transplant follow-up, and ongoing management of patients with complex medical needs.

Key Responsibilities
  • Coordinate patient care from pre-transplant evaluation to post-transplant follow-up, including scheduling appointments, coordinating testing, and communicating with patients and their families.
  • Develop and implement individualized care plans for patients undergoing renal and pancreas transplantation, taking into account their unique medical needs and circumstances.
  • Collaborate with the multidisciplinary transplant team, including surgeons, nephrologists, and other healthcare professionals, to ensure seamless care coordination and optimal patient outcomes.
  • Provide education and support to patients and their families, addressing their concerns and questions about the transplant process and post-transplant care.
  • Participate in quality improvement initiatives to enhance patient care and outcomes, and contribute to the development of policies and procedures related to transplant care.
Requirements
  • Graduate of an approved school of nursing with current registration in the state of Massachusetts.
  • Minimum two years of experience in renal medicine-focused patient care, with a strong background in transplant coordination.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a strong commitment to providing high-quality patient care.
