Job Description:
We are seeking a dedicated and compassionate Home Health Registered Nurse to join our team. In this role, you will provide high-quality care to patients in a behavioral health setting.
Responsibilities:
- Assesses new patients within 8 hours of admission
- Provides individual nursing care as ordered by the physician
- Manages medication administration including monitoring and storage daily
- Participates in interdisciplinary formulation of individual treatment plans
- Dictates and writes progress notes for one to one & group sessions in accordance with policies and procedures
- Assists in coordinating risk management activities to reduce or eliminate patient/staff/visitor injuries, illnesses, death, and/or financial loss
- Completes and signs necessary admission paperwork and other required documentation with guidelines mandated by law and applicable hospital policies and procedures
Skills Needed:
- Organizes and prioritizes work while being cost-effective and conserving resources
- Able to work with multi-disciplinary team
- Organized and self-disciplined
- Thorough documentation
- Utilizes listening skills that indicate understanding and promotes accurate interpretation of other's concerns, motivations, and feelings
- Works toward resolution of interpersonal conflicts as they arise
Qualifications:
- Valid Registered Nurse (RN) license in Louisiana; CPR, CPI, and BLS required
- Minimum of 2 years of nursing experience, preferably in behavorial health
- Strong clinical skills and knowledge of home health care practices; Excellent communication and interpersonal skills
- Knowledgeable on use EMR systems
- Ability to work independently and manage a flexible schedule
- Reliable transportation and valid driver's license
