Responsibilities:

  • Evaluate patients and conduct diagnostic and prognostic tests
  • Provide physical therapy services as prescribed by the attending physician
  • Instruct agency visit staff, patients, and families on therapy techniques and home exercise programs
  • Participate in developing and revising the physician's plan of care
  • Complete clinical/progress notes and documentation as per agency policy

Qualifications:

  • Graduate of an accredited Physical Therapy Program
  • Current Registered Physical Therapist License
  • Current State Driver's License
  • Current Auto Liability Insurance according to company policy
  • Current CPR Certification
  • 1 year of experience with geriatrics (preferred)
  • Associates or higher degree
