As a Home Health Occupational Therapist, you will have the opportunity to work with patients in their own homes, providing personalized therapy and support to help them achieve their goals.
Job Summary:We are looking for a highly motivated and compassionate Occupational Therapist to provide high-quality care to our patients. As a Home Health Occupational Therapist, you will be responsible for assessing patients' needs, developing and implementing individualized therapy plans, and educating patients and their families on how to perform daily tasks and maintain their independence.
Responsibilities:- Assess patients' physical and cognitive abilities to develop personalized therapy plans
- Provide occupational therapy to patients in their homes, focusing on activities of daily living, mobility, and safety
- Collaborate with other healthcare professionals, including RNs, LPNs, and PTs, to ensure comprehensive care
- Document patient progress and adjust therapy plans as needed
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care
- Graduate of an accredited Occupational Therapy program
- Active OT license in Michigan
- Minimum of 1 year of occupational therapy experience, preferably in home healthcare
- CPR certification
- Knowledge of state and federal home health regulations
