Provides counseling services in individual and group sessions. Therapeutic efforts are primarily directed towards addressing issues related to recovery, self-esteem, relationships, and grief. Maintains appropriate professional boundaries with all persons involved. Serves as a case manager for assigned adult clients and their children. Provides crisis intervention and conflict resolution assistance. Conducts recovery related educational sessions and groups providing information and instruction. Collaborates with direct care, parenting, self-sufficiency and trauma providers to ensure holistic services are provided. Collaborates with all involved staff in the provision of care to ensure continuity and maximum effectiveness of care/services provided. Participates in the admission process for assigned adult program participants utilizing a compassionate and empathetic manner to ensure all clients entering the program feel welcome, appreciated and understood. Helps ensure all clients feel a focus on collaboration and engagement from the very beginning of their involvement with House of Mercy. Completes initial and ongoing assessments including an evaluation utilizing the American Society of Addiction Medicine Patient Placement Criteria (ASAM). Performs a comprehensive bio-psychosocial assessment to analyze and synthesize the client’s status and addresses the client’s strengths, problems, and areas of clinical concern. On an ongoing basis assesses client needs/strengths, behaviors, and progress toward desired outcomes. Specifically monitors client progress in areas of personal growth, education, prenatal care, parenting, employment, living skills, etc. Determines need for outside professional evaluations (e.g., mental health), and independently refers to in-house and community practitioners/agencies as needed. Participates in the development of treatment plans in accordance with program requirements, state regulations, etc. and communicates pertinent treatment plan information to other staff as needed. Updates and revises client treatment goals/objectives on a regular basis consistent with House of Mercy and regulatory expectations. Participates in regularly scheduled care meetings to discuss/update staff on client progress, status, and treatment needs. Professionally communicates with other program staff to promote team collaboration, input, and involvement in problem solving/resolution insuring consistency in the implementation of treatment plan goals/objectives. Effectively communicates client service needs with community agencies, schools, courts, service providers of significant others, and mental health/other health care professionals to provide an extended, high quality continuum of care. Collaborates with treatment team in developing appropriate discharge plan and aftercare programming. Assists clients in their efforts to locate and obtain appropriate housing post House of Mercy. Integrates the resident into appropriate post-placement programs including Alumni and Continuing Care groups. Prepares and distributes required reports and other documentation of services according to House of Mercy policies and procedures, applicable laws, regulations and licensing requirements. Responsible for the maintenance of client files. Assures all needed releases of information and other paperwork are present in the file and up-to-date. Promptly requests new or additional information and paperwork as needed. Responds to volatile situations by mediating to resolve issues and to model appropriate crisis intervention and problem solving techniques. MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED: Bachelor’s degree from an accredited four-year program, required. Degree in a human services related discipline, preferred. CADC Certification Valid Driver’s/Chauffeurs License required, must meet Mercy’s Motor Vehicle Safety Standards, must be at least 18 years of age and be eligible to drive per Iowa state law. Proof of completion of Mandatory Reporter abuse training specific to population served within thirty (30) days of hire. Ability to understand medical terminology and communicate effectively with healthcare professionals.
