EDUCATION

  1. Education: Graduate from an accredited school of nursing (RN) required, bachelors preferred.
  2. Population Specifics: Knowledge and skills necessary to provide care appropriate to the population served in RM Department.
  3. Experience: Two (2) or more year's clinical experience preferred.

Acute care or community social agency case management experience preferred.

Must possess good assessment, intervention and problem solving skills. Must be able to address physical, cognitive, emotional, financial and psychosocial capabilities of the patient/family. Able to develop and maintain collaborative and professional relationships with multiple stakeholders (patient/family, nursing staff, physicians, payers, and external agencies). Ability to understand medical and social work terminology and principles. Ability to communicate in an effective and efficient manner, verbally and in writing. Basic computer skills (Outlook, Word, Excel, and databases) a must. Ability to meet required deadlines while maintaining expected quality. Good organizational skills, with the ability to establish priorities and switch focus with frequent interruptions.

  1. Required Certification: Case Management Certification (CCM) preferred.
  2. Required License: Washington State Registered Nurse (RN) license, Washington State Drivers License with liability insurance to use personal vehicle and/or LHN Company vehicles for LHN business purposes.
  3. Other: Bilingual and/or English-Spanish speaking abilities preferred. Ability to work with culturally diverse population.

LOURDES HEALTH NETWORK REQUIRED RESPONSIBILITIES/COMPETENCIES

  1. Responsible for becoming knowledgeable and compliant with Department, Hospital, Fire/Safety/Disaster, Educational requirements and Personnel Policies.
  2. Follows the Corporate Compliance Program and maintains confidentiality standard.
  3. Demonstrates support and adherence to the Mission and Values Statement of the Sisters of St. Joseph of Carondelet as an agent of Lourdes Health Network.
  4. Participates and supports the Continuous Quality Improvement process and provides optimum customer service.
  5. Responsible for development and implementation of self-management skills.

Salary Range: $42.59 - $59.63
