Position Summary

The Case Manager Supervisor is responsible for the supervision of Care Managers and the Case Management Specialist. The Supervisor coordinates the onboarding of members to the SOURCE program, the assignment of Care Managers to members, the initial and ongoing delivery of case management services to each member as well as the suspension and discharge of members from the program. The Supervisor maintains oversight of the case management operations to ensure members are provided with the highest quality of care. This position applies skills in monitoring records and charts, in preparing written reports, and in maintaining effective working relationships with others. The Supervisor also plays a key role in the hiring and termination of resources that support the operations of the Case Management Department.

Primary Duties and Responsibilities

  • Must represent the Ace brand by presenting a professional appearance and attitude while engaging internal and external customers.
  • Responsible for the overall day to day operations of the Case Management Department.
  • Responsible for the administrative and staff functions of the Case Management Department.
  • Prepare and present the Case Management report at the Weekly Ace Meeting (W.A.M.).
  • Foster a stress-free work environment for Care Managers (CMs), Case Management Specialist and peers by working as a team player, identifying problems and furnishing resolutions.
  • Ensure adequate and consistent communication amongst CMs, Medical Director, Billing Personnel, Administrative Personnel, Compliance Personnel, Administrator, Assessment Personnel, Intake Personnel and the Executive Team.
  • Monitor and assist CMs in reaching their monthly goals and targets.
  • Delegate the number of members to be seen by CMs within a certain time period and the set deadlines for completion of those tasks.
  • Monitor CMs calendar for changes and completion of scheduled work as assigned weekly.
  • Ensure all the required documentation are completed in entirety by all Care Managers in the course of their day to day duties.
  • Responsible for being involved in the hiring and termination process of CMs.
  • Random check of CM work to ensure quality and adherence to timelines.
  • Oversee and handle internal and external communications for the Case Management Department.
  • Responsible for conducting the prescribed provider meeting process.
  • Oversee and ensure the completion and follow up of EDWPs.
  • Ensure all approved packets are sent to providers within a timely manner.
  • Oversee the management of the member issues log.
  • Ensure Care Managers are following up and resolving issues in a timely manner.
  • Review monthly contacts, case notes and quarterly visits documentation ahead of being sent to the compliance department for final review.
  • Provide customer service satisfaction to members, providers, and all other stakeholders in a courteous and professional manner.
  • Keep current with the policy and guidelines stipulated by the Elderly Disabled Waiver Program.
  • Prepare and conduct the weekly IDT meeting by tabling and finalizing all approvals, service increases, service decreases, member issues, discharges and transfers.
  • Responsible to sign-off on all quarterly visits (QVs) weekly in the supervisor capacity.
  • Responsible for signing off on all documents that require a supervisor signature.
  • Assist in the management of the pending members list to prevent non-compliance.

Work Environment

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

Required Education and Experience
  • Bachelor's degree in Social Work, Human Services or Psychology.
  • Prior management skills one year or longer is a MUST.
