Responsibilities:

- Develop and maintain collaborative relationships with area treatment providers

Provide assessment, early engagement, and referral to those clients with co-occurring substance abuse diagnoses

Complete substance abuse assessment for all clients admitted to program

Participate in treatment planning with clients with substance abuse related goals

Work with clients in obtaining needed resources and other general case management activities

Act as member of the Individual Treatment Team for identified clients

Regularly attend and participate in daily organization staff meeting

Participate in on-call rotation as assigned

Requirements:

Full certification as an addictions counselor or a co-occurring disorders professional by a statewide certification body

Experience working with adults with serious and persistent mental illnesses

Valid driver's license and ability to drive company vehicles
