The Team Member We Are Searching For
The ideal Licensed Vocational Nurse (LVN) is passionate about delivering superior, patient-centered care and is an excellent communicator with all types of patients. Under the direction of the Director of Patient Care, the Licensed Vocational Nurse (LVN) provides services to patients in accordance with the State Nurse Practice Act, agency policies, and accepted professional standards of practice utilizing a comprehensive base of nursing theory and nursing process and communicates and documents observations and assessments.
What you'll need to accomplish:
- Assures employees receive complete and accurate directions and information about patient needs.
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Uses health assessment data to determine clinical diagnosis and treatment plan.
- Provides health care instructions to the patient as appropriate per assessment and plan.
- Receives and carries out physician orders.
- Notifies patient physician, the Director of Clinical or Designee and/or Clinical Supervisor immediately of any changes in the patient's condition.
- Completes documentation (paper or automated system).
- Supports the verification of staff time records with schedules.
- Provides appropriate notification of schedule to employee, patient, contract agency, supervisor, and others as appropriate.
- Responds to emergency calls and arranges schedule accordingly.
- Includes the patient and the family in the planning process.
- Initiates appropriate preventative and rehabilitative clinical procedures.
- Educates the patient and family in meeting clinical and related needs.
- Ensures patient's privacy, home, and medication safety.
What do we require from you?
Minimum Qualifications
Licenses/Certifications:
- Current license as a Licensed Vocational or Practical Nurse in the State of California required
- Current CPR certification required
Experience:
- Minimum of one (1) year of nursing experience required
- Home health care experience is strongly preferred
- Excellent communications, both written and verbal, organizational, presentation and facilitation skills
- Ability to use a computer, copier, and facsimile machine.
- Ability to speak, read, and write English
- Bilingual (English/Spanish) a plus
- Self-directed with the ability to work with little supervision
- Excellent observation skills
- Flexible and cooperative in fulfilling role obligation
- Maintains confidentiality in all aspects of the job
- Respects the confidentiality of information in patient and employee records
- Schedule flexibility is necessary to meet the needs of patients as well as the leadership team
