As a Home Health Licensed Vocational Nurse, you will provide high-quality care to patients in their homes, working collaboratively with a team of physicians, RNs, CNAs, and aides. Your responsibilities will include taking patient vitals, conducting assessments, documenting progress, and reporting changes to the RN supervisor. You will also assist the RN in carrying out the patient's plan of care and teaching self-care techniques to the patient and their family. Additionally, you will educate the family on the patient's plan of care, medication, and home safety, and collaborate with the interdisciplinary team to coordinate patient care and plan for discharge. To be successful in this role, you will need to have a graduate degree from an accredited vocational nursing program, an active LVN license in California, and a minimum of 1 year of home-health experience. You will also need to have CPR certification and any other necessary certifications.
