The mission of Faith Regional Health Services is to serve Christ by providing all people with exemplary medical services in an environment of love and care. As a provider-led multispecialty healthcare team, Faith Regional Physician Services is committed to delivering high-quality, comprehensive services to the people of Northeast Nebraska.

Under limited supervision, independently provides treatment, assessment, education, counseling and case management or equivalent activities within his/her age specific competencies to individuals with mental health or addiction problems. Maintains appropriate documentation, will track completion of documented interventions/outcomes, and performs other related work as required. Excellent organizational skills are mandatory. Deliver services while demonstrating Faith Regional Health Services Mission, Vision and Values.

Essential Job Duties and Responsibilities:

1. Demonstrates ability to appropriately modify approach and procedures to meet needs of age/diversity of population served for the following age groups. Must be able and willing to see Pediatric/Children and Adolescent patients.

  • Incorporates cultural considerations in the provisions of care.
  • Knowledge of growth and developmental stages.
  • Considers life changes/effects on health beliefs and behaviors.
  • Provides necessary safety measures.
  • Provides information and involves family/caregiver in decision making.
2. Performs all responsibilities/duties required by the Behavioral Health Department as defined in the scope of practice, to assure that the unique nature of the patient is addressed. This includes but is not limited to the age of the patient served.
  • Provides treatment following an evidenced based therapeutic approach which uses counseling techniques to deal with thoughts, feelings, behaviors in the treatment of mental or emotional disorders and maladjustment's to relieve psychological distress, attain insight into the nature of the individual's disorder, and/or assist the person in decision making and acquiring new behavior within the scope of professional certification, established age specific competencies and according to departmental policies and procedures.
  • Conduct assessments and diagnose mental health disorders as well as develop personalized treatment plans based on individual's treatment needs
  • Documents essential actions taken in the treatment of mental health and/or addiction issues for the purposes of confirming and communicating treatment delivery data by recording information related, but not limited to, assessments, treatment plans, referral/placement, progress notes and discharge summaries.
  • Maintains excellent oral and/or written communication with referral sources in order to provide continuity of patient care through information distribution regarding patient progress and program information
  • Identifies methods/issues to improve the clinical outcome of services in order to enhance the quality of care by participation in formal and informal conferences, staff meetings, and committees.
  • Maintains current knowledge in the professional field by participating in seminars, academic endeavors and continuing medical education offerings.
  • Provides consultation/collaboration with other clinicians and treatment team members regarding individual plans of care.
Essential Organizational Duties and Responsibilities:

2. Maintains current knowledge/certification/licensure per organization policy. Pursues professional growth and development.
  • Practices within legal boundaries according to organization policy and procedures.
  • Attends continuing education programs in order to maintain licensure. Attends organization sponsored training programs as required.
  • Maintains certifications as determined by department/clinic policy.
3. Functions as role model for current and new staff.
  • Facilitates problem resolution among coworkers as observed by management and peers.
  • Demonstrates a constructive approach during all interactions with coworkers and management toward the organization.
  • Promotes the profession of healthcare workers in the community.
  • Presents neat appearance in proper attire and identification as required by the position, department and the organization.
  • Continuously displays a positive attitude within the department/clinic and across departmental lines to contribute to the overall customer service program.
  • Serves in committees and assist with programs of the hospital to constantly improve the quality of health care and promote good public relations.


4. Completes all required safety training programs and can describe his or her responsibilities related to general safety, department/service safety, and specific job-related hazards.

5. Understands and adheres to Faith Regional Health Services Corporate Compliance Plan, Personal Conduct Policy, Grievance Process, Confidentiality, Safety & Security, Infection Control, HIPAA and other departmental and organizational policies, procedures and guidelines.

6. Must be able to remain awake and alert during working hours. Physical presence at facility is required during scheduled working hours to perform job functions. Work in a timely manner, within the time allotted for the shift per census count.

7. Performs other duties as assigned.

Hours will be dependent on patient census and workload. This is a 40 hour a week position.

Job Requirements:

The requirements listed below must be representative of the knowledge, skills, minimum education, training, licensure, experience, and/or ability required.

Master's Degree required.

Current Licensed Independent Mental Health Practitioner (LIMHP), Licensed Mental Health Practitioner (LMHP), or Provisional Licensed Mental Health Practitioner (PLMHP) from the state of Nebraska required.

Previous Experience Requirements:

EXPERIENCE:

Previous clinical experience preferred.

Previous healthcare experience preferred.
