Responsibilities:

  • Evaluating and providing physical therapy services to patients as prescribed by the attending physician
  • Instructing patients and families in home exercise programs and use of assistive devices
  • Participating in the development and revision of the physician's plan of care
  • Completing clinical/progress notes and documentation per agency policy

Qualifications/Licensure:

  • One (1) year experience preferably in a hospital or skilled nursing facility
  • Graduate of an accredited Physical Therapy Program
  • Current Registered Physical Therapist License
  • Current State Driver's License
  • Current Auto Liability Insurance according to company policy
