TITLE: CARE CENTER Case Manager
REPORTS TO: DIRECTOR CARE CENTER
SALARY: HEALTH AND SOCIAL SERVICES
JOB GOAL
The CARE Center Social Worker will provide services to students and families which strengthen home/school/community relationships and alleviate barriers to learning.
This is a grant-funded position with funding through May 30, 2029. Applicants for this position should be aware that this role works on a 12-month schedule and requires some availability for weekends and evenings.
Desired Qualifications:
- Master's Degree in Social Work (MSW) or related degree preferred
- One year of experience working with families and school-aged children
- Knowledge of community resources available for students and families
- Demonstrate knowledge of the Social Work Code of Ethics and sensitivity to diverse cultures
- Ability to fluently speak, read, and write English and Spanish
- Serve as a liaison between Chandler CARE Center, schools, families, and community agencies;
- Identify cultural, ethnic, financial, physical and emotional challenges which might be negatively impacting students' success and provide support to mitigate barriers;
- Attend meetings and participate in coalitions, collaborations, and workgroups as requested by the Director;
- Collaborate with CUSD schools and grant team;
- Implement evidence-informed, prevention programs and practices that teach positive behaviors, promote social-emotional development, and ensure school climates conducive to learning;
- Collect, review and analyze data to support grant reporting and program needs;
- Consult with school personnel regarding problems affecting students' academic progress, behavior, and personal well-being;
- Alleviate family stress and provide interventions and support to enable students to function more effectively in school and the community;
- Assist families in accessing and utilizing school and community resources;
- Continue professional growth to maintain knowledge of skills related to job functions and timely topics;
- Perform other related duties as assigned;
- Perform all duties in a safe and prudent manner as directed.

Physical Requirements - Office
