Care Partners is seeking an Intake Coordinator to join the administrative support team to assist clinical staff in processing hospice and palliative referrals, creating patient records, and ordering supplies for patients and families upon admission. As the Intake Coordinator with Care Partners, you play an important role on our team, in our community and for our clients.

You will handle administrative tasks to facilitate the processing and coordination of referrals and admissions. This will also include completion of required documentation for onboarding new patients. Care Partner's goal is to provide support for the comfort and health of patients and families consistent with hospice philosophy at end of life.

Your Responsibilities:

  • Complete the administrative onboarding for new admissions through initiation of referral in electronic medical record (EMR) and coordinate with clinical admission staff
  • Negotiate changes in admission schedules that meet patient and referral source needs
  • Collect key referral contact information, enter data from admission checklist into EMR, obtain supporting documentation, procure durable medical equipment (DME) as needed, and initiate orders as authorized
  • Process insurance verifications for all admitting patients
  • Assist case managers and clinical staff by adding appointments to staff calendars for initial assessments
  • Follow up with patients/families and referral sources to assure that admission processes have been completed and patient needs have been met
  • Assist in completion, coordination, and scheduling of patient transfers and traveling patients in accordance with our organization's policies and state/federal regulations
  • Audit electronic medical records for accuracy, timeliness and insurance and regulatory compliance needs
  • Assist Quality by reviewing and submitting Hospice Item Set (HIS) data to Centers for Medicare and Medicaid Services (CMS)
  • Troubleshoot and solve problems occurring in the electronic medical record while onboarding new patients in collaboration with EMR staff personnel
  • Utilize a website to coordinate delivery and pickup of Durable Medical Equipment and work with customer service to resolve any issues that arise
  • Work in collaboration with other departments on strengthening relationships with referral sources and providing updates on current admission openings
  • Provide clerical support for clinical leaders when not engaged in admission coordination such as answering phones, obtaining medical records, and other clerical areas as assigned
Qualifications:
  • Certified as a Medical Assistant is highly preferred or equivalent education and experience preferably in a hospice setting
  • High level of customer service experience
  • Education or experience in the medical field with a strong working knowledge of medical terminology and systems
  • Must have strong clerical and administrative skills to work with applications such as Word and Excel.
  • Must have demonstrated experience working independently with minimal supervision while delivering effective and on-time results
  • Requires the ability to handle a variety of tasks simultaneously while maintaining professional composure and positive approaches, including demonstrated organization and prioritization skills to set priorities and manage workload
  • Excellent oral, written, and interpersonal communication skills
  • Must be reliable and dependable in completing assignments within deadline, with strong attention to detail and accuracy in results
  • This position requires the ability to stoop and bend and to lift up to 25 pounds

Schedule: Usual hours fall within the 8-5, M-F, schedule. Requires in-office hours for initial training and orientation as determined by the supervisor for this role. Work may be performed through a combination of in-office (minimum of three days) and remote work to be determined between the employee and manager following a thorough training and onboarding.
