Job Summary
The case manager facilitates the plan for an orderly transition of patients and works closely with physicians and payors to ensure proper use of fiscal resources.

Education and/or Experience

  • Graduate from a CCNE or NLN accredited nursing program is strongly preferred. Bachelor's degree in nursing preferred. Four years related experience and/or training; or equivalent combination of education and experience preferred.

Licensure/Certification/Registration

  • Licensed as a Registered Nurse and professional certification preferred.
  • Membership in a professional organization preferred.

Work Schedule

  • Varied Shifts; 80hrs/2wks

Pay
The hourly range for this position is $31.00 - $46.50 per hour.
