As a Behavioral Health Specialist, you will:

• Provide evidence-based treatments as well as work with other mental health providers, when indicated

• Assists with more complex cases, and will participate in inter- and intra-departmental teams, projects and initiatives

• Prepares an approved care plan in partnership with the RN care manager/treating physician

• Mobilizes resources to achieve expected clinical outcomes within the desired timeframe

A day in the life:

• Ability to perform psychiatric and substance-abuse disorder assessment, coordination, implementation, case planning, monitoring, and evaluating to promote effective use of community resources and health benefits

• Interfaces with Medical Directors, Physician Advisors and other multi-disciplinary team members on the development of treatment plans

• Consults with medical providers regarding patient concerns

What success looks like in this role:

• Carries out a review of comprehensive diagnostic evaluations, treatment plans and functional assessments, coordination, implementation, case planning, monitoring, and evaluating patient outcomes in an effort to optimize outcomes

• Able to carry out care coordination activities on assigned caseloads to ensure physical, social and behavioral needs of patients are being met

• Coordinates and expedites patient discharge planning initiatives from the day of admission through the day of discharge

• A personal commitment to a person-centered approach, along with strong relationship-building, communication, and active listening skills required

Requirements

We are excited about you if you have these things:

• Role requires a master's degree in social work, counseling, or a related health field

• Current active, unrestricted license in Wisconsin (LMHC, LCSW, LMFT or LMSW), to practice as a health professional within the scope of licensure in applicable states

• Willingness to obtain licensure in additional states, as necessary

• Minimum of 4 years of experience with facility-based and/or outpatient psychiatric and substance abuse or substance-abuse disorder treatment; or any combination of education and experience

• Experience in either utilization management, case management, or with a broad range of experience with complex psychiatric/substance abuse cases preferred

• Knowledge of state and federal rules, regulations and accrediting bodies

• Knowledge of Diagnostic and Statistical Manual of Mental Disorders (DSM) V or most current diagnostic edition

• Prior telephonic behavioral health experience is a plus

• Bilingual is a plus

• This position requires a hybrid work arrangement, in a Madison, WI office
