Job Responsibilities:
Provide therapy services following CPT and facility goals, ensuring high standards of care.
Handle necessary administrative duties for the rehab department's daily operations.
Build strong relationships with patients, their families, and third-party payers, ensuring effective communication and collaboration.
Qualifications:
Graduate of an APTA-approved physical therapy program or equivalent.
Licensed or certified to practice in Illinois or Wisconsin as a physical therapist.
Adhere to the APTA Code of Ethics and Standards of Practice.
Your Work Schedule:
We are looking for experienced professionals available to work Monday to Friday, 7 a.m. - 6 p.m., across various locations, providing services in:
Memory Care.
Assisted and Independent Living.
Homecare and Outpatient Services.
