We are seeking a highly skilled and compassionate Physical Therapist to join our Outpatient Rehabilitation Services team at Northwestern Memorial Healthcare. As a Physical Therapist, you will play a vital role in helping patients recover from injuries, illnesses, and surgeries, and achieve their optimal level of function and independence.
Responsibilities:- Conduct thorough evaluations and assessments of patients to identify their physical therapy needs and develop personalized treatment plans.
- Provide evidence-based physical therapy interventions, including exercises, modalities, and education, to help patients achieve their goals and improve their quality of life.
- Collaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure comprehensive and coordinated care.
- Communicate effectively with patients, families, and caregivers to educate them about their treatment plans and progress.
- Document patient progress and outcomes in a timely and accurate manner, using electronic medical records and other tools.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to ensure high-quality patient care.
- Stay current with best practices and research in physical therapy, and apply this knowledge to improve patient outcomes and advance the field.
- Master's or Doctoral degree in Physical Therapy from an accredited program.
- Current licensure as a Physical Therapist in the State of Illinois.
- Basic Life Support (BLS) certification.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong analytical and problem-solving skills, with the ability to assess patient needs and develop effective treatment plans.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
