Job Summary

As a Medical Care Coach, you will be part of our Complete Care Program. You will play a vital role in our healthcare organization by providing personalized support and guidance to patients to help them navigate the complex healthcare system and improve their overall health and wellness. Commonwealth Pain and Spine is looking for passionate, tech savvy team members to serve our patients who are enrolled in Medicare's Chronic Care Management Program In this role you will work closely with patients to empower them to make informed decisions about their healthcare, manage chronic conditions, and achieve their health goals. This role requires a deep understanding of medical care, strong phone communication skills, and a compassionate approach to patient support.

Job Duties and Responsibilities

Patient Engagement:

  • Establish a strong rapport with patients, promoting trust and open communication.
  • Assess patients' needs, preferences, and health goals, and create individualized care plans.
  • Schedule and conduct regular check-ins to monitor progress and provide ongoing support.
Health Education:
  • Educate patients about their medical conditions, treatment options, and self-management techniques.
  • Provide guidance on medications, lifestyle changes, and preventive care.
  • Implement and improve patients' plan of care by updating medications, appointments due, biometrics, symptoms, and interventions made.
  • Offer resources and information to promote health literacy.
Care Coordination:
  • Collaborate with healthcare providers, including physicians, nurses, and specialists, to ensure comprehensive patient care.
  • Schedule and coordinate appointments, tests, and follow-up care as needed.
  • Facilitate communication between patients and their care team.
  • Close gaps in care by encouraging and assisting with preventive care measures (ex: annual well visits, vaccines, cancer screenings, follow-up/specialist appointments, etc.).
Empowerment and Motivation:
  • Support patients in setting realistic health and wellness goals.
  • Monitor progress toward goals and provide positive reinforcement and motivation.
  • Assist patients in overcoming barriers to achieving their health objectives.
Data Management:
  • Maintain accurate and up-to-date patient records and care plans.
  • Document all patient interactions and interventions in compliance with relevant regulations.
  • Generate reports and track patient outcomes to measure program effectiveness.
Patient Advocacy:
  • Advocate for patients' needs and rights within the healthcare system.
  • Assist patients in navigating insurance, financial assistance, and community resources.
Compliance and Training:
  • Stay current on healthcare regulations, industry trends, and best practices.
  • Attend training sessions and participate in continuous learning and development.
All other duties assigned including duties performed for affiliates, assigns, lessees, contractors or other third parties.

Requirements

Required Skills & Abilities
  • Strong understanding of medical terminology, healthcare processes, and chronic disease management.
  • Excellent interpersonal and communication skills.
  • Empathetic and patient-centered approach to care.
  • Organizational and time management skills.
  • Ability to work independently and as part of a healthcare team.
  • Proficiency in electronic health records (EHR) and healthcare software.
  • Compliance with all healthcare privacy regulation practices.
Experience
  • Previous experience in healthcare coaching or care coordination.
  • Previous experience in a call center preferred.
Education
  • LPN Certification Required
