Job Summary and Qualifications

The Physical Therapist (PT) delivers high-quality, patient-centered care by managing the comprehensive therapeutic process for an assigned group of patients. Collaborating closely with physicians and the patient care team, the PT completes assessments and develops personalized care plans that focus on achieving patient goals and enhancing patient experiences.

Responsibilities include:

  • Conducting evaluations and implementing treatment interventions within legal and ethical guidelines.
  • Assessing therapeutic needs and contributing to the patient's total plan of care.
  • Delivering skilled therapeutic care in patients' homes to promote improvement in condition and functionality.
  • Teaching patients, families, and caregivers about therapy techniques, exercises, and equipment to be utilized outside of therapy sessions.
  • Prioritizing caseloads and coordinating daily staffing and assignments.

Qualifications:

  • Active Physical Therapist license.
  • Valid Driver's License.
  • Bachelor's Degree in Physical Therapy or related field.
