Responsibilities

Provide case management, home or field visits, coordinate services for program participants, and assist in program activities and administrative support. Act as lead to support Case Managers with job duties.

  • Enroll participants based on program eligibility criteria
  • Document case management using the program templates (Service Plan, Case Notes, Referral Log, Contact Log, Consent Forms, and other forms as directed)
  • Monitor the coordination of services and documents additional needs identified for each Re-Entry participant
  • Provide appropriate referrals for reentry population (housing, employment, public benefits)
  • Encourage and monitor that participants complete the parole requirements
  • Maintain information on available resources and services in the county and region conducive to reduce recidivism
  • Maintain electronic client files as defined by the program
  • Protect client confidentiality following program protocol by utilizing encrypted email and/or fax, and obtain consent to release information for communication with other agency partners including Parole Office and Substance Use Disorder agencies
  • Administer surveys, screenings, or assessments as defined by the program
  • Participate in weekly reflective supervision meetings with Program Supervisor/Program Manager
  • Contribute to program protocol review and modifications
  • Communicate participant incidents to Program Supervisor/Program Manager before action is taken
  • Respond to chart audit recommendations and complete by deadline
  • May organize and present educational sessions with participants as needed
  • May attend community network and collaborative meetings as assigned to include Parole office, Sober Living and substance use disorder programs
  • Participate in outreach activities or provide a program presentation to agency partners
  • Participate in weekly and monthly staff meetings
  • Complete required weekly and monthly reports
  • Enhance professional growth and development through participation in educational programs, current literature review, in-service meetings, workshops, and training defined by the program
  • Support the vision, mission and goals, and demonstrate commitment to the values of the organization
  • Perform other duties as directed
Qualifications Minimum
  • High school graduate or equivalent with five years case management experience, or equivalent combination of education and experience; 
  • Valid California driver’s license, current vehicle insurance, and reliable transportation
  • Bilingual English/Spanish
  • Availability and willingness to work evening and weekend hours, as required
Preferred
  • Bachelor's degree in related field 
Required Skills/Knowledge/Abilities
  • Excellent verbal and written skills necessary for communication with patients/clients, providers and other staff
  • Ability to interface with all levels of personnel in a professional manner, including people of all social, cultural and ethnic backgrounds and within the constraints of government funded programs and those community members facing justice involvement
  • Ability to prioritize and meet deadlines
  • Ability and willingness to meet the organization’s attendance and dress code policies
  • Ability to handle confidential materials and information in a professional manner
  • Excellent customer service skills and commitment to providing the highest level of customer satisfaction
  • Experience/familiarity with computers, business e-mail, communication systems and internet search capabilities and proficient in Microsoft Office products, specifically Word and Excel

Pay Range

  • $21.00 to $25.00 DOE
