CASE MANAGER DUTIES:

  1. Answer all incoming emails and phone calls in a timely manner (within 24 hrs.)
  2. Make sure database is constantly updated on all cases
  3. Start the process of the new match once file/case once received from directors
  4. ESCROW ESTABLISHMENT-Send the referral for the client escrow account to
  5. Greet the Client and the Surrogate and/or Egg Donor by sending an intro email.
  6. Request initial deposit from the Clients .
  7. PSYCH SCREENING-Once the funds are received by escrow, it’s time to start for Surrogate to schedule their “mandatory” screening appointment. Once you have confirmation of the scheduled appointment, notify the clients and continue to next step. Send a request to escrow to pay the psychologist and mark off on check off form. Scan in all documents and put in each client’s folder.
  8. MEDICAL SCREENING- Prepare clinic referrals. Send the medical referral to the IVF nurse coordinator and cc yourself so you can save the email in the corresponding professionals folder. Update the check off sheet. If you do not receive notice of the appointment date/time, follow up with the clinic in 3-4 days to inquire. Be sure to also contact the Surrogate and/or Egg Donor and get updates from them directly and add your communications in Daylite.
  9. TRAVEL (IF NEEDED) Help the Surrogate and/or Egg Donor with travel arrangements, for her medical screening (if needed).
  10. LEGAL CONTRACTS Send legal referrals to both the SUrrogate's and the IP’s selected attorneys..
  11. Submit all invoices to escrow for payment once the screening appointment or legal review is complete.
  12. Once the Surrogate and/or Egg Donor have signed legal contracts with the IP’s and you receive legal clearance, email the IP’s and request the initial funding deposit be wired to escrow.
  13. Once the IVF or egg retrieval cycle dates are given to you by the clinic, email the IP’s and provide encouragement and support and wish them luck
  14. EGG DONOR/SURROGATE INSURANCE-Assist IPs to find suitable insurance policy to cover their journey. Send referral to insurance broker to meet with IPs.
  15. SURROGATE SUPPORT TEAM WELCOME-Welcome new Surrogates onboard and provide orientation
  16. SURROGATE REIMBURSEMENT FORMS-Process Surrogate reimbursement forms submitted by Surrogates via email

The Case Manage/Support Team at SAI is viewed as the heart of our company and has the unique opportunity to play a role in all journeys that take place at SAI. Attending special moments like the embryo transfer, heartbeat ultrasound or a delivery, are significant and momentous moments for both the Surrogate and the Intended Parent(s). A lot of time, effort and patience went into getting to the "BIG DAY" and we rely on this department to stay up to date on all important milestones during each journey. This department is responsible for maintaining constant communication with our Surrogates and Intended Parents.

Keep the Screening, Cycling, Pregnant and Delivered Lists updated weekly as these lists are used by other departments and must be as current as possible. Update database as well with ANY changes as that is checked much more frequently.

Keep the PBO list updated and interact with the attorneys on facilitating this process.

All Case Managers are responsible for applying for and obtaining Health Insurance for their own Non-Insured Surrogates during Open Enrollment (OE) by working with our Insurance Agent.

They must keep a list of Surrogates needing insurance coverage, confirm current coverage and determine with the Insurance Agent if it's beneficial for them to switch insurance or not.

Always put emails (and attach files, including cycle calendars, pregnancy confirmation forms, releases, etc. ) and notes from phone calls or texts in database so that anyone looking at a file has the most current information and email the Intended Parents after speaking with their Surrogate to give them an update

Case Management is responsible for hosting and attending Surrogate retreats. SAI hosts three retreats per calendar year and all staff members participate on a rotation basis.
