Case Manager - Bilingual Spanish (Preferred)

Reports to: Lead Case Manager
Hours: Full-time, exempt position with flexibility.

Responsibilities: The Case Manager is responsible for coordinating case management services. The Case Manager's tasks shall include development of intake assessment, individual service plan, family reunification and discharge. The Case Manager shall also ensure that all services provided to each child are properly documented in each child's case file. Final approval for hire will be done by the Project Officer of the Office of Refugee and Resettlement which is a sub-agency of the Department of Health and Human Services of the federal government.

Essential Functions:
1. Works with a multifunctional team to maintain well-being and safety of minor and to ensure that services are being provided
2. Maintains cultural sensitivity to work effectively with diverse populations of minors and coworkers
3. Compiles, prepares, maintains, and submits documentation in accordance with ORR and DPW guidelines
4. Identifies a sponsor and assess for suitability based on requirements for safe release
5. Document progress that families and sponsors make toward risk reduction, achievement of service goals and positive case outcomes
6. Completes assessments within allocated timeframe to maintain compliance
7. Coordinates release of minor and facilitates referrals as needed
8. Participates with weekly meetings with ORR representatives to provide feedback and progress on cases
9. Participates in team, supervisory, and other program meetings as needed
10. Utilizes the assistance of Case Aides to achieve program goals
11. Willingness to work evenings, weekends, and holidays as needed, as well as participate in an on-call rotation
12. Models and assists in the implementation of the positive behavior management techniques, such as HART Model and Trauma-Informed Care
13. Ensure confidentiality in all program operations and service population
14. Uphold professional and ethical standards of conduct outlined in the DS employee handbook

Requirements:

Master's degree preferred or a bachelor's degree in the behavioral sciences, human services, or social service fields and 3-5 years of progressive employment experience in the human service fields.

Previous case management experience preferred.

Must have oral and written fluency in English and Spanish.

Must be proficient in Microsoft Office Suite and Adobe.

Must have Act 33, 34 and FBI clearances.

Child welfare and/or case management experience is strongly encouraged.
