Acts as a liaison between the patient, family/caregiver, physician, and other members of the healthcare team
Engages in multidisciplinary collaboration to ensure continuity of care and quality outcomes
Utilizes a patient-centered approach to assess for health-related needs, identify barriers, develop individualized care plans, and facilitate referrals to appropriate internal and community resources
Provides chronic disease-management to reduce utilization, prevent readmissions, and improve outcomes
This position requires expertise in the nursing process, using critical thinking skills to plan and coordinate care
The Care Coordinator will also be responsible for conducting pre-visit screening and patient communication prior to scheduled Health Maintenance Visit
Requires the knowledge to assure documentation integrity and complete and accurate billing and coding
Contributes to a positive working environment and performs other dutiesas assigned or directed to enhance the overall efforts of the organization
