As a Patient Care Manager (PCM) you have the responsibility and accountability with the Care Center Clinical and Medical Directors for planning, organizing and directing of all activities related to patient-centered care, fostering nursing excellence and the retention and recruitment of staff on a designated unit in the care center. Essential Job Functions Uses a broad theoretical knowledge base and the principles of patient centered care to assist in the establishment of patient centered care standards. Ensures that standards of patient centered care are maintained. Demonstrates clinical and leadership skills, serving as a role model for staff members. Communicates current trends, concepts, and issues of professional practice. Maintains skill in clinical specialty area. Recognizes and identifies clinical practice problems and facilitates solutions. Implements established staffing patterns based on patient needs, staff qualifications, and resources available to ensure quality patient centered care. Maintains continuity on unit by adjusting staffing based upon patient need promotes, has knowledge of and utilizes appropriate patient care protocol relating to the physical, psychosocial and developmental needs of neonatal, pediatric, adolescent, adult, and geriatric patients. Assists in the development, implementation, and monitoring of the budget within established guidelines. Monitors the selection, use and control of the equipment and supplies of the unit. Participates in recruitment and selection of unit staff. Demonstrates 24-hour accountability for maintaining appropriate staffing levels. Schedules staff in accordance with patient care needs and union contracts. Evaluates staff performance on a formal and informal basis. Complies with standards and regulations of various licensing, regulatory and accrediting agencies. Demonstrates awareness of and compliance with hospital policies and procedures. Participates in development and review of policies and procedures. Serves on appropriate care center and organizational committees. Creates a work climate that encourages positive staff morale, motivation, and commitment for all patient care team members. Applies interpersonal communication skills to maintain an open network of communication among staff members, care center and sector leadership, and other health care professionals and departments. Facilitates regularly scheduled staff meetings to provide opportunities for discussion of staff and unit concerns. Collaborates with Discipline chiefs and department heads in appraising the performance of personnel aligned with the unit. Contributes to the Patient Services Sector by offering constructive suggestions for change and participating in the planning, implementation, and evaluation of new and existing programs. Formulates, implements, and evaluates unit goals consistent with Care Center goals and with the Hospitals' mission and Transformation. Develops, implements and evaluates unit goals through active participation in committee work and task force assignments as needed at the unit, Care Center or Sector levels. Provides and supports an environment which encourages staff to accept responsibility for learning as part of the development of their own professional practice. Participates in the planning and evaluation of educational programs to meet unit needs and to promote staff development. Develops, implements, and evaluates the unit-based orientation program for new employees. Collaborates with Clinical Nurse Specialist. Assists in assuring that all staff are provided the opportunity to attend mandatory educational programs, in-services, and competency days per JCAHO regulations. Serves as a role model for staff by presenting or arranging for educational programs. Collaborates with the Patient Services Support Center/Academy Continuing Education Committee to plan, implement and evaluate nursing education activities.
