Job DescriptionIn conjunction with the Lung Nodule Screening Program the Navigator oversees daily operations and workflow for the Lung Nodule Screening Program. Collaborates with the providers and clinical staff to support all efforts related to guiding patients through the screening process. The Navigator works with referring physician offices and clinics to manage initial referrals through follow-up imaging tests. The Navigator supports all aspects of the Lung Nodule Screening Program database including data entry, statistical reporting, and enhancements including participating in community outreach events.Responsibilities The Navigator directs patients through the screening, enrollment and appointment process. In conjunction with the providers and staff, supports all efforts related to guiding patients and families through the screening process. Schedules appointments and diagnostic studies, monitors prior authorizations and tracks status of patients in database. Performs program Quality assessment checks, audits, and reviews. Develops and drives initiatives to eliminate barriers to the lung cancer screening program. Establishes processes and standards that ensure consistent patient scheduling including follow up diagnostic imaging exams. Communicates with referring physicians regarding patient appropriateness for program and coordinates information with referral services Performs Patient Follow up Management for patients already screened, uses database to ensure appropriate follow up appointments are scheduled and kept as recommended. Distributes patient letters, provides follow-up and confirmation calls. Responsible for Database Management - manage all aspects of the Lung Screening database including inputting patient screening data, patient letters, data entry input of all patient management activities, statistical reporting. Works closely with Radiology IT with database design, enhancement and maintenance. Prepares weekly program statistics. Provide updates on program activity, statistics and recommendations for program enhancements. Is an available resource for the Cancer Committee. Coordinates weekly with providers and staff to review program status. Coordinates with Patient Financial Services and the Finance Department as needed. Prepares analytical data as requested. Recommends and implements program scheduling improvements. Helps plan and organize community outreach events, offsite work and hours may occasionally apply. Provides cross-coverage to the cardiology and pulmonology departments as needed.Qualifications Education: Associates degree preferred Licensure, Certification & Registration: A minimum of 3 years of progressive relevant experience in a healthcare environment. Previous experience in Pulmonary Medicine, Oncology or Diagnostic Imaging preferred. Skills, Knowledge & Abilities: Strong customer service skills required. Ability to take initiative and work independently with minimum supervision. Knowledge of computer systems, Microsoft office- Word, PowerPoint and Excel. Strong organizational and interpersonal skills. Excellent communication skills required to interact with all levels of Clinic personnel.
