Led by a team of highly trained medical and therapeutic professionals, we partner with families to develop a personalized, effective care plan to make the best decisions for the child. Family input is valued and considered every step of the way. This continuum of care endeavors to help each child reach his or her maximum functional ability medically, emotionally, socially, and spiritually.

If you choose a career at CHC, you will Complete an initial needs assessment and act upon critical needs appropriately and immediately. Develop a comprehensive resident-driven individual personal plan (IPP) with both short-term and long-term goals identified; create a timeline and measures for each goal. Monitor progress toward goals in regularly scheduled interdisciplinary meetings; adjust as needed. Maintain resident files to include conversations, progress toward goals, and documentation of any incidents. Assures the safety of each resident through frequent tours of the facility, inspecting for any identifiable hazards. Document a monthly Q-note for each assigned resident. Administer a quarterly evaluation of all assigned residents. Implement and evaluate programs within the active treatment plan. Maintain relations with family/guardians and case managers at healthcare facilities, purchase and inventory resident clothing for assigned caseload. Provide input on staff performance as requested. Update families and guardians about the resident's overall status monthly and as needed. Oversee staff adherence to scheduled case management services and activities. Conduct employee in-service training when requested. Act as a resident advocate during the planning, development, and implementation of treatment and individual education plans (IEP). Serve on committees as assigned. Participate in quality improvement activities.
