Position Function:

To support and serve youth and their families by providing and coordinating service and system navigation to meet the identified needs. The Resource Navigator will respond to all referrals in a timely manner. The Resource Navigator provides a strengths and needs assessment and connects youth and family to needed services in the community through an ongoing developing understanding of state and federal service offerings. The job requires sensitivity to the service population's cultural and socioeconomic characteristics and direct collaboration with other organization personnel in an off-campus site.

Education Requirements:

A bachelor's degree in social work, psychology or related human services field from an accredited college or university is preferred. An additional year of experience working with families and youth in case management/service coordination may be substituted for a year of college (30 hours) to secure a bachelor's degree. Must be fluent in speaking, reading, and writing in Spanish.

Experience Requirements:

The position requires one year of case management or involvement in connecting families in federal or state service navigation such as WIC, TANF or regulated service system. Understanding of crisis intervention and trauma informed care interventions, good verbal and written communication skills are required. Possess a working knowledge of basic computer skills.

Functional Requirements:

Requirements include providing expedient and ongoing assessment, service navigation, case management, and transition planning for all assigned families. Maintain accurate and timely documentation of services for all assigned clients. Prepare and submit all required documentation, reports, and statistical data in a timely manner. Follow policies and procedures of ACH and represent the agency in a positive and professional manner in any co-located service agency location. Coordinate the collaboration of referral agencies, family members, and other support systems to meet the needs of the family. Effectively communicate with any agencies associated with the referral agencies complying with HIPAA and law. This position provides on-call support for Safe Place®.

Working Conditions:

Exposure to children/youth with behavior problems, including possible physical aggression. Exposure to illness and blood. This position offices in shared spaces within other agencies, requiring sensitivity to other agencies' procedures and policies.

Exposure to Confidential Information:

Maintain confidentiality and follow policies related to HIPAA and control of client information and any personnel records.

Key Expectations/Responsibilities:

Programming:
  • Collaborate on a team of co-located service providers to address family's needs.
  • Utilize expertise and awareness of state and federal services.
  • Partner with state and federal funded entities to connect families to services with up to the date information on access and qualifications and establish formal Memoranda of Understanding between them and ACH.
  • Collaborate with families utilizing a strengths and needs assessment to help identify family's areas of resilience and areas of challenge.
  • Maintain effective engagement strategies to establish participation of program participants in needed service acquisition.
  • Provide families the opportunity to identify and address their own needs and ideas of how to overcome identified challenges.
  • Facilitate and document assessment and service planning for all assigned program participants.
  • Complete all documentation in a timely manner.
  • Document and assess program participant progress in database.
  • Provide skills training and/or support groups,
  • Provide crisis response when necessary, assessing for safety and needed intervention.
  • Offer services after normal business hours, on holidays and during assigned on call rotation.
  • Participate in ongoing training and supervision to develop identified skills that meet strategic goals, contractual obligations, professional growth, and individual targets.
  • Participate in staff meetings to share resources to aid other staff in meeting the needs of families in fulfilling the mission and vision of ACH.
  • Provide support to families in developing discharge and aftercare planning.
Community Relations
  • Collaborate with other service providers and community organizations to share and provide information.
  • Represent ACH in a professional and positive manner.
  • Demonstrate awareness of community issues and priorities.
Documentation and Reporting
  • Maintain up-to-date case records.
  • Develop and document strengths and needs assessment.
  • Provide regular case management services for each assigned program participant.
  • Documentation of case progress notes.
  • Communication and coordination of needs for each client.
  • Assist in development of program related forms and measurement tools.
  • Complete a discharge and aftercare plan for each program participant.
