Job Title: Intake Specialist and Resource Navigator

Catholic Charities and Community Services is seeking a highly motivated and compassionate Intake Specialist and Resource Navigator to join our team. In this role, you will be responsible for providing initial intake services and broad resource navigation services to individuals experiencing homelessness who are seeking shelter services at the Samaritan House.

Key Responsibilities:

  • Connect with individuals new to shelter life to ensure they are assessed and receive the necessary support.
  • Review referrals from community partners and assess readiness for Samaritan House program.
  • Accurately and promptly complete all required documentation/data, including entering relevant participant information in daily logs.
  • Communicate with other staff to resolve problems with individual participants.
  • Assist in the development and implementation of case management services that help participants become more independent and functional.
  • Conduct in-take and orientation of new participants.
  • Schedule and complete all shelter intakes.
  • Network within the community resources to achieve the best services available for the participants.
  • Advocate with the participants when they have barriers to overcome in meeting their goals.
  • Maintain confidentiality of participant and agency information.
  • Use creativity and innovation in program development and service delivery.
  • Complete HMIS 101 and HMIS 201 training and accurately enter and maintain HMIS enrollments, updates, and dismissals.
  • Provide HMIS Agency Administration duties, such as entering data elements, and generating HMIS reports and consistent monitoring to ensure data quality.

Qualifications:

  • Ability to understand the challenges faced by special need populations in the community.
  • Capacity to identify participant barriers to activities of daily living.
  • Ability to use computer to enter participant's information into case files and compile program statistics.
  • Aptitude to assist participant in identified barriers.
  • Knowledge of the service population's cultural and socioeconomic characteristics and the appropriate techniques and practices for the participant population.
  • Ability to interact effectively as a team member and independently with Agency staff and with a diverse participant base.
  • Knowledge of Motivational Interviewing.

Education and Experience:

Bachelor's degree (BA/BS) from accredited four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
