Provide medication therapy management services in collaboration with other providers to assure that drug therapy is safe, appropriate, cost-effective, and efficient. The pharmacist must maintain competency in unique aspects of drug therapy for the patients whom they provide care for, including factors related to patients of all age groups (neonates, pediatrics, adults, and geriatrics) and patient disposition (inpatient, ambulatory, and outpatient). Pharmacy services provided may include interdisciplinary patient care rounds, medication reconciliation, medication therapy monitoring, medication orders management, medication dispensing, patient education, drug information, drug dosing, and various scholarly activities. The pharmacist will provide both clinical and distributive tasks for safe medication use, and provide training and education to students and residents as necessary. Mastery of the Cerner pharmacy and hospital information system application is required. Must be proficient with technology and workflows associated with the medication management process including (but not limited to) automated dispensing cabinets, anesthesia carts, USP IV requirements, and clinical decision support software. The pharmacist ensures that patient care is maintained through compliance with the department's and organization's policies, rules and regulations by the respective State Board of Pharmacy, and other applicable regulatory bodies.Responsibilities:Essential Job Functions1. Patient Care ServicesReview medication orders for appropriate dose, duration, frequency, dosage form, indication, allergy, interactions, and contraindications. Ensure that medication regimen is consistent with all organizational medication use guidelines and policies, or other evidence-based guidelines in the absence of organizational guidelines, which includes (but is not limited to): antimicrobial use, intravenous to oral transition, renal dosing, anticoagulation, and pharmacokinetics/serum drug concentration protocols.Adjusts medication regimen based on approved protocols and/or consultation with interdisciplinary team.onitor medication therapy for potential or actual drug interactions (e.g. drug-drug, drug-disease, drug-allergy, food-drug, or contraindications) and outcomes.Works as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient-centered careParticipates in interdisciplinary patient care roundsand oversees pharmacy resident and student participation in patient care rounds when applicable.Performs and/or reviews medication histories , comparing prescriber's orders to the patients active medication list(s), and works with prescribers to make changes as necessaryDocument activities related to medication reconciliation in medical record across patient's transition of care (including but not limited to: admission, transfer, and discharge)Complete discharge medication review facilitate the discharge process, including discharge prescriptions, and/or provide discharge education per department procedures or upon request by patient or health care provider.
