Dispatch Health is recruiting a DispatchHealth Activation Liaison (DAL) who will be responsible for supporting our Provider Partners to improve patient activation and utilization of Dispatch Healths services. The DAL must be an accountable and motivated collaborator between DispatchHealth and their Provider Partners. The successful candidate must employ an entrepreneurial spirit and embody a sense of urgency around desire to increase utilization of our services. Our solutions are broad and clinically complex, so the ideal candidate must be clinically minded, collaborative, and able to communicate effectively to patients, providers, Partner Leadership, and DispatchHealth leadership. DispatchHealth offers a spectrum of innovative services, and this role will enable our partners to activate patients into the service they need most efficiently and effectively. The company is pursuing a rapid expansion plan through 2025, and the DAL role will be critical to driving the company's success. This role is in the foundational stage of development and this role offers the opportunity to work closely with DispatchHealth Commercial leaders in the hopes to expand this /team across more markets and partners. This position reports to XYZ and will be remote with travel expectations throughout the markets within their area of responsibility along with Denver headquarters for key meetings.
*Must be in located in FL between Tampa/Orlando/Daytona/Ocala*
What You'll Do:- Responsible for becoming an expert on DispatchHealths services, as well as market/partner specific workflows, to properly identifying which patients align to which services and how best to initiate onboarding within their specific market
- Coordination with onboarding teams/functions to ensure swift hand-offs and maintain focus on identify patients and relationship building with market partners/providers
- Provide ongoing training to market partners inclusive of DH services, patient identification, patient education, and partner specific patient activation.
- Lead Coordination on communication, data, and process improvement efforts between patients, partners, market ops, and growth team
- Lead all types of patient activation efforts by educating providers and patients on the DispatchHealth clinical model and ensuring proper care requests are made.
- Facilitate acute care activations to support PCP practices and other locations such as ALFs, outpatient care management centers, nurse triage locations, to support partners sourced volume growth with patient identification, education, and activation,
- Facilitate discharge support activations by identifying eligible patients during inpatient rounding, educating patients/families, and coordinating scheduling prior to discharge.
- Support the Emergency Department (ED) to Home program by identifying patients, providing education, and completing bedside scheduling before patient departure.
- Participate in health system communication strategies, including providing training on using DispatchHealth onboarding tools (Dispatch Express and EMR order integration via Redox).
- Engage with ED front desk and clinical teams to identify patients leaving without being seen or leaving against medical advice, offering Acute Care or ED-to-Home visits.
- Foster partnership engagement, ensuring integrations with frontline teams, identifying opportunities for process improvements, and supporting ongoing evaluation and success metrics for visit requests and completions.

- Experience building trust with senior-level executives at health care organizations and technology
- Strong executive presence and highly developed cross-functional communication and collaboration
- Clinical Experience: MA, DHMT, Case Manager, or similar clinical experience Advanced understanding of payor strategies and deal structures that align interests and drive mutual
- Ability to work from Partner facilities 3-5 Days per week
