Summary: Care Coordinators provide administrative office support to the case management team in the Minnesota Army National Guard in support of Individual Medical Readiness. Non-clinical case management administrative personnel will be experienced in all aspects of job performance and qualified in their respective functional areas. Hourly Range: $34.22 to $34.22 per hour Responsibilities include: Tracking medical status of ALL assigned Soldiers as delegated by the case manager, including tracking scheduled appointments; documenting communication efforts with the Soldier, their unit, and the appropriate military provider; and collecting medical information for review by the case manager or provider. Participating in case reviews as needed to ensure that every Soldier with an open case is being tracked and managed. Assist Soldiers in scheduling appointments with appropriate medical/dental facilities. Communicate with delegated government personnel to track compliance. Monitor designated medical readiness databases to ensure all scheduled services are completed/updated and current medical status is reflected. Assisting the Case Managers by putting together and processing the different types of packets as well as entering and tracking the patient information. Receiving, communicating, and processing the medical and personnel records that are submitted in all the various e-systems. Requirements: Education/Experience - Must have a minimum of a high school diploma or equivalent and have at least two years’ experience in an administrative position, preferably medical. Security – Must be able to pass a national security background check for access into military electronic systems. Must be proficient in MS Office Suite and general administrative duties.
