Job Description Position Title: Home Health, Registered Nurse FLSA STATUS: Non-Exempt REPORTS TO: Manager of Clinical Practice DEPARTMENT: Home Care COMPANY: OVNA Workdays are Friday, Saturday, Sunday, Monday POSITION SUMMARY : The primary function of this role is to conduct admission and resumption of care assessments with a focus on developing a patient specific plan of care. The RN in this role provides quality and safe delivery of nursing and home health services that meets the organization’s mission of providing exceptional patient care. PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS : The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned. + Assesses patient’s health problems and develop patient specific plan of care. + Provides physician ordered skilled nursing services. + Completes 6 points daily which can be a combination of SOC and ROC’s. + Assess appropriateness of patients at SOC to ensure they meet the following criteria that are justifiable for services: homebound, identifiable skilled need and focus of care. + Has a clear understanding of oasis and plan of care development. + Educates patients and families on patient specific plan of care. + Collaborates with the physician, home health team members and other care providers, within and outside agency to enhance patient care. + Attending case conference as requested. + Adheres to policies and procedures. + Participates in quarterly QAPI meetings. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
