Primary responsibilities include Intake and data entry of complete and accurate referral information and providing clerical support to the Intake Department.ResponsibilitiesObtains patient referral data from referral sources via phone or fax.Perform clerical duties as requested.Documents initial referral information.Identifies and documents patient team assignment.Notifies intake coordinator of pending referrals.Enters new patient information into computer system as needed.Acts as liaison between Intake and referral sources with initial referral information.Assists Intake nurses with faxing and copying and distribution as needed.Assists Intake with phone communication as needed.Assures accurate transfer of patient insurance information to IVC.Performs in accordance with system-wide competencies/behaviors.Performs other duties as assigned.Skills & AbilitiesTeam playerGives high priority to client satisfaction and customer needs.
