About the Position

A dedicated and compassionate Patient Care Coordinator will ensure the smooth and efficient scheduling of any offsite referrals/diagnostic imaging and procedures.
  • Promotes a positive work environment and contributes to a dynamic team-focused work unit that actively helps one another to achieve optimal departmental and organizational results.
  • Serves as a liaison between patient, facility, and physicians to ensure that exams, tests, and procedures are scheduled promptly, accurately, and appropriately. Communicates special requests and add-ons with appropriate department team as needed.
  • Communicates with departments throughout the facility when barriers to the current schedule present.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Schedule appointments for Diagnostic Imaging
  • Schedule appointments for Outside Referrals and fax necessary records
  • Schedule appointments for procedures with outside facilities or hospitals
  • Schedule and arrange any peer 2 peer needed for offsite imaging or procedures with providers and clinic MA.
  • Follow all outside referral process to meet Quality Measures
  • Provide exceptional customer service by addressing patient inquiries, concerns, and requests promptly and professionally.
  • Maintain accurate and up-to-date scheduling records, ensuring proper documentation and adherence to established protocols.
  • Collaborate with the imaging department staff to optimize patient flow and ensure timely completion of appointments.
  • Follow HIPAA regulations and maintain strict confidentiality of patient information.
  • Communicates with all appropriate parties regarding patient and procedure.
  • Follows-up and ensures all documentation is obtained.
  • Verifies if all authorizations have been obtained prior to scheduling procedures.
  • Inform and provide patient with necessary information and instructions prior to procedure date.
  • Provide high quality support by phone, email, and other written communications.
  • Other duties as assigned.
Qualifications:
  • High school diploma or equivalent
  • Previous experience in a healthcare setting, particularly in scheduling or administrative roles, is desirable.
  • Familiarity with diagnostic imaging procedures and terminology is a plus.
  • Strong organizational and multitasking skills, with the ability to prioritize tasks effectively.
  • Excellent communication and interpersonal skills to interact with patients, physicians, and staff members.
  • Proficiency in computer systems and medical scheduling software.
  • Ability to work in a fast-paced environment while maintaining attention to detail and accuracy.
  • Ability to work independently by guiding oneself with little or no supervision and depending one oneself to get things done.
  • Exceptional customer service and phone etiquette
  • Ability to communicate professionally with patients.
