Description

The Patient Navigator with the Women's Health Center will assist patients and their families in managing through the vast array of services and programs offered by the hospital and the community to meet their health and wellness needs. The Patient Navigator will provide personalized, high touch care, advocating the integration of whole person health of body, mind, and spirit.

Required qualifications:

  • Bachelor's Degree in Social Work
  • Upon hire: National Provider BLS - American Heart Association

Preferred qualifications:

  • 1 year of experience in health care setting

About the Team

Providence Physician Enterprise (PE) is a service line within Providence serving patients across seven states with quality, compassionate, coordinated care. Collectively, our medical groups and affiliate practices are the third largest group in the country with over 11,000 providers, 900 clinics and 30,000 caregivers.
