Description

Responsible to provide instruction, support, and supervision to individuals during Assisted Day Services. Acts as the primary liaison for clients receiving services and their parents, guardians, referral, and funding sources.

Requirements

1. Promotes, encourages, and supports the creation of opportunities for people with disabilities to achieve goals and connect in the community to become as independent as possible.

2. Protects and safeguards all confidential information about its clients, propriety information on Aspiro and its operations. Confidential information should only be shared on a 'need to know' basis to perform their job.

3. Facilitates the development, monitoring and evaluation of Person-Centered Plans and Positive Personal Profiles for clients receiving services.

4. Attends and is an active member of IEP (Individual Education Plan) or MCP (Managed Care Services) teams that establish Individual Service Plans.

5. Seeks input from and provides support and consultation to staff regarding Person-Centered Plans for clients receiving services.

6. Provide instruction, assistance, and supervision for clients in areas such as daily living, leisure time activities, recreation, sports, swimming, socialization, and all other areas identified in their Person-Centered Plan.

7. Works with staff to assist people receiving services, their families, guardians, and significant others to organize and communicate what is important to them. Promotes, encourages, and supports the development and evaluation of individualized goals based on a person's strengths, preferences, and needs.

8. Provides direct care, instruction, support, and supervision to people receiving services, as needed.

9. Assists with transportation needs of clients including providing supervision and assistance for clients departing and boarding buses and other vehicles, driving Aspiro vehicles, which includes operation of wheelchair lifts.

10. Follows safe lifting practices of clients to and from wheelchairs or from reclining positions to sitting and/or standing. Operates automated lift equipment and utilize gait belts as needed.

11. Responds to alarms and emergency notices and assists clients to evacuate the building or move to designated shelters according to written evacuations plans.

12. In the absence of the nurse, administers medication to assigned clients.

13. Performs other incidental and related duties as required and assigned.

Qualifications:

Education:

High School Diploma/GED required. Bachelor's Degree in human services or related field or equivalent experience preferred.

Experience:

Experience with people with disabilities or another human service group preferred.

Driver's License:

A valid Wisconsin driver's license is required with coverage under the Aspiro auto insurance plan approved.

Personal Vehicle:

A personal vehicle is required and must have insurance that meets Wisconsin's minimal requirements.

Hours:

Typically, Monday-Friday, 7:30am-4:00pm

Knowledge, Skills, Abilities:

Microsoft Office computer skills, excellent verbal and written communication skills, and an ability to work with diverse individuals and groups. Flexible and adaptable to changes.
