Job Title: Patient Care Advocate
Job Description
Seeking a dedicated Patient Care Advocate to work closely with members and providers to enhance the overall experience by closing care gaps and removing barriers to care. This role involves direct outreach, face-to-face interaction, and collaboration with large IPA and/or group practices to improve HEDIS measures and ensure compliance with State and Federal standards. The advocate will act as a liaison between members, families, physicians, and facilities, providing education, scheduling necessary appointments, and connecting members with community resources. This position requires maintaining strict confidentiality of business and protected health information.
Hard Skills
+ 3+ years of nursing experience (RN, LPN, HEDIS, etc.)
+ Valid NJ license
+ Own car for transportation
+ Experience with medical terminology
+ Knowledge of HEDIS measures and ICD-10/CPT coding
Job Type
This is a Contract-to-Hire position with a duration of 30 Weekly.
Work Site
This is a hybrid position requiring some days on-site in New Jersey and some days remote.
