This Care Coordinator is responsible for supporting referred clients in accessing comprehensive knowledge and access of community resources and benefits and boosting client self-sufficiency through an understanding of community agencies and support. This position will collaborate with on-site caseworkers to provide new arrivals with light case management services via access to legal services, individual coaching, and connection to resources, benefits, and workshops pertaining to housing, education, and the health system in the U.S. as needed. The position focuses on creating stability and uses a strengths-based case management approach to address client needs and promote long-term self-sufficiency by offering a more robust understanding of rights in the U.S. This position operates within grant-funded programs, requiring specific services to be provided within set timeframes and appropriate documentation of all activities. The role is part of a larger integration team and is responsible for the success of their referred cases, as determined by the client experience, program outcomes, timeliness and accuracy of reporting, and collaboration with colleagues.

Role & Responsibilities

    • Provide cultural and language appropriate services
    • Provide comprehensive, client-centered legal service navigation to referred clients, including but not limited to: Ensuring clients understand basic U.S. law;
    • Under the supervision of an attorney, provide assistance with filling out Lautenberg-related Affidavits of Relationship (AORs) and other immigration applications;
    • Identify immigrant rights and community advocates;
    • Enroll clients in "Know Your Rights," immigration workshops, housing, healthcare, and education workshops offered at WRS
    • Work with cross-departmental staff to ensure client integration and referrals across programming;
    • Support and build ongoing partnerships with local service providers and community organizations addressing client understanding of U.S. laws and the role of law enforcement;
    • Secure safe and affordable housing along with initial move-in costs;
    • Ensure clients understand good tenant practices and responsibilities;
    • Assist with emergency assistance as needed;
    • Identify tenant rights and community advocates;
    • Provide individual coaching to promote self-sufficiency in independently navigating housing laws and practices
    • Enroll and support in accessing education for school-aged children within the family;
    • Ensure clients establish advocacy within the district for their students;
    • Assist with understanding the types of programs within the district that may meet the unique needs of their students;
    • Provide adults with referrals to continuing education and/or ESL/ VESL opportunities
    • Enroll clients in WRS workshops on navigating the education systems in America
    • Provide transportation and interpretation, when necessary, to required appointments
    • Assess participant needs, strengths, and goals and then establish comprehensive action plans with short and long-term goals
    • Provide comprehensive, client-centered health navigation services to referred clients, including but not limited to: Enroll and support in accessing all eligible insurance and healthcare benefits, including MediCal and Social Security;
    • Ensure clients establish primary care and have access to all needed specialty services;
    • Assist with booking healthcare appointments and scheduling transportation and interpretation through insurance;
    • Identify appropriate service providers to meet client health and wellness needs, successfully coordinate client referrals, and assist and advocate for ongoing access to services and care;
    • Provide individual coaching to promote self-sufficiency in independently navigating the healthcare system
    • Complete timely and accurate case notes, track program data, maintain up-to- date knowledge of program requirements, and ensure compliance with case files and documentation;
    • Document participant services and activities thoroughly and in a timely manner in accordance with program and WRS requirements
    • Update and maintain client information in protected, online databases (Dynamics, Teams, etc.);
    • Conduct regular, recurring meetings with clients to ensure progress on goals and to provide appropriate connections to resources.
    • Achieve grant requirements and deliverables through timely, high-quality services;
    • Participate in on-going training to improve service delivery and ensure compliance;
    • Review and regularly present the status and progress of each case to the Program Manager;
    • Collaborate successfully across WRS staff and departments, as well as with external service providers to ensure holistic case management;
    • Attend meetings as instructed by supervisor;
    • Other duties as assigned.
Knowledge, Skills, & Abilities

    • Proven ability to utilize digital communication tools, such as Zoom, MS Teams, and other platforms;
    • Ability to work as part of a team and support program-specific volunteers and interns;
    • Proven success achieving goals and working effectively with all levels of staff in a fast-paced, multi-cultural environment;
    • Self-starter with excellent problem-solving skills combined with the proven ability to multi-task, prioritize duties, and manage time effectively;
    • Highly self-motivated and comfortable working independently towards case-specific, department, and office-wide goals.
