Job Description

The Team Coordinator is a key liaison between patients and their Patient Centered Medical Home Team, including providers, clinic staff, and other health care professionals. Their primary function is to provide leadership for a seamless experience of care for patients calling their health care team, providing answers to patients' questions, or connecting the patient with another team member, tracking clinic quality measures and facilitating outreach to patients regarding services that are due. The Team Coordinator must understand complex health systems, and help patients navigate the system to meet their healthcare needs.

Required Qualifications:
  • 2 years experience working as a Patent Relations Rep in PMG
  • Experience with electronic medical records
Preferred Qualifications:
  • 1 year working with a diverse population

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

About the Team

Providence Clinical Network (PCN) is a service line within Providence serving patients across seven states with quality, compassionate, coordinated care.
