Essential Duties and Responsibilities:
+ Assesses health care trends in communities
+ Collaborates with emergency response planning and trainingwhen applicable
+ Works with public health officials to help communitiesaccess and receive care
+ Provides immunizations and health screenings to all populationsin the community
+ Provides patient and community education
+ Monitors and delivers high-quality patient care
+ Records and analyzes medical data in various communities
+ Evaluates the health of patients and develops treatmentplans in collaboration with medical providers and multidisciplinary teammembers
+ Participates in the evaluation and documentation of patientsand care
+ Assists in developing, evaluating, and implementingprofessional development education
+ Refers patients to other providers as needed
+ Manages budgets of public health facilities if applicable
+ Reviews the work of assigned team members for thoroughness,soundness of recommendations, and compliance with the standards, policies,procedures, and regulations of the CDC as well as the County Department ofPublic Health
+ Performs other duties as assigned/necessary
Minimum Requirements:
+ Active Nursing Licensure in the state practicing
+ Two (2) years of public health or community experiencepreferred
+ Complies with all relevant professional standards ofpractice
+ Participation and completion of Amergis' Competency programwhen applicable
+ Current CPR if applicable
+ TB Questionnaire, PPD or chest x-ray if applicable
+ Current Health certificate (per contract or stateregulation)
+ Must meet all federal, state and local requirements
+ Successful completion of new hire training as applicable tojob site
+ Understand patient confidentiality and HIPAA requirements
+ Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required
+ Computer proficiency required
+ Must be at least 18 years of age
