POSITION SUMMARY

The Community Impact Specialist is responsible for interfacing with community systems in the geographic location. Collaborates with county community partners. Maintains documentation related to outreach and collaborations via CRM. Works with developing and maintaining community relations and community partners. Maintains accurate documentation of center of influence interactions and community relations. Coordinates, facilitates, and fosters communication when developing agency relationships with community partners, which includes but is not limited to, Community Coalitions, Health Homes, Sister agencies, Specialty providers, local hospitals, clinics, law enforcement, fire and EMT services.

Supports development and implementation of programs. Works with Community Medical Services clinic staff to establish best practices when receiving referrals from outreached agencies or community partners. Will work with clinic staff, community partners, and Maricopa Pima County leadership to make all efforts to secure placement and funding for all pima County individuals identified as in need in a manner consistent with CMSs mission, vision, and values. The Work will be performed in a wide range of settings, including any CMS clinic location, community-based outpatient setting, community agencies, correctional health facilities, and in transport vehicles.

PRIMARY JOB RESPONSIBILITES

- Attends meetings and interfaces with significant team members related to improvement of coordination of care for OTP

clients. Responsible for building and being the integral contact for coordination of care between OTP clients and community

agencies.

- Provides documentation of all possible client/agency encounters.

- Maintains Community Relations documentation/ log for all contracts, meetings, conversations, presentations, and other

outreach activities. Maintains appropriate documentation of coordination of care efforts. Will maintain data in multiple data

bases such as but not limited to: QuickBase ,QuickBase, Methasoft and CRM for all outreach programs.

- Maintains appropriate coordination of care with all systems and provides documentation as needed/ requested from multiple

system partners.

- Attends required meetings: Clinical staff meetings, clinical supervision, workshops, seminars, and training, etc. as assigned

by, Regional Director, Community Programs and Integration Manager, Clinical Coordinator, or Chief Executive Officer.

- Community Based Services that include providing program information and assessment information to non- clients. Providing

clinical support, including assessments, to special populations and projects. Completing weekly outreach or as necessary

around surrounding clinics located in Maricopa county.

- Attends Provider Meetings and other meetings within the community where they exist. Attends community meetings for

purpose of advocating, educating, and supporting system partner relationships.

- Attends staffing or Adult Recovery Team meetings as needed.

- Supports program development and implementation. Works with project and program management team.

- Other duties as assigned

- Maintain transportation logs will send completed logs to supervisor by the 3rd of each month.

- Knowledge of community resources including but not limited to; food stability, housing, recovery stability, employment,

benefits including medical, social security, food stamps etc.. Inpatient MAT friendly recovery services, shelters,

emergency services, PCP, and special populations.

- Receiving referrals for Community Support from multiple CMS clinics and community system partners, will be responsible for

tracking all referrals from PSS, clinicalPSS, clinical staff and or community partners this will include documenting outcomes of referrals in appropriate systems related to referral.

- When completing home visits, or community outreach is required to be done in a team of two or staffed with supervisor.

- Complete documentation with 24 hours of shift, if not completed must notify supervisor.

- Will adhere to CMS policy and procedures, as an approved driver will report any driving infractions immediately to supervisor.

- 45% productivity required.

QUALIFICATIONS:

- Experience in substance abuse rehabilitation and Opioid Treatment Programs is desirable.

- Bilingual a plus. Ability to positively and appropriately interact at meetings interact at meetings positively and appropriately

with all level of employees from outside agencies, including executive team and management employees.

- Effectively follow up and training capabilities in order to address required changes or areas for improvement.

- Ability to observe and care for clients and ability to interact and develop a team environment.

- Ability to travel within the region most of the time- including all CMS locations when needed. May include client transportation

when needed

- Basic to intermediate computer knowledge

- Problem solving, conflict resolution, and strong customer service skills

Tools and Equipment Requirements

- The ability to use a phone, computer, printer, and copier is required.

- Frequent use of Microsoft office products, including but not limited to Outlook, Word, Excel, and PowerPoint.

- The ability to use the internet and various web browser software is required.

Physical Working Conditions and Office Setting Description

- Requires sitting and standing associated with a normal office environment.

- Manual dexterity using a calculator and computer keyboard.

- Requires prolonged sitting, standing, frequent bending, stooping, or stretching.

- Some lifting may be required.

- Frequent and prolonged typing and frequent and prolonged operation of computer, keyboard, and telephones required.

- Requires occasional use of fax machines, telephones, copiers, and other office equipment.

- Employee must regularly lift and/or move up to 10 pounds, frequently lift and/or move up to 25 pounds and occasionally lift
