IHCS is seeking an experienced case manager with a
Bachelors/Masters in Social Work
to facilitate integrated care in the areas of living, learning, working and socialization by assessing client needs, evaluating treatment options, creating service plans, coordinating care, linking to services, and reviewing case progress to promote safety and well-being. The ideal candidate will work closely with multi-disciplinary team members such as physicians, APRNs, mental health and other health care professionals, as well as family and friends of clients to ensure clients are supported and can achieve the best outcomes. Case managers will coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered. Case managers will assist clients with improving their overall functioning through identification of strengths, weaknesses, abilities and preferences in order to attain future.
This position will also require the candidate to provide case management internally as well as provide case management services to chronically homeless population within our community. The individual will coordinate service needs with the outreach team.
Responsibilities
Conduct ongoing, and often long-term, street outreach, engaging with unsheltered homeless individuals and families;
Maintain a caseload of 35 active clients;
Conduct intake and assessments to determine case goals to overcome barriers for people experiencing homelessness and substance use/co-occurring substance use and mental health disorders;
Conduct SPDAT assessments for Housing and Coordinated Entry (CE) assessments;
Participate and enroll within the Client Track HMIS for CE and Outreach services;
Provide housing navigation services, assisting with transportation and system navigation skills to access documentation necessary to qualify chronic homeless individuals and families for housing. Navigation may include: transporting clients and walking alongside them to obtain birth certificates, ID’s, Medical Clearances, income verifications, HUD McKinney forms, and mental health evaluations;
Provide referrals and help clients maintain contact with services for disability applications and employment;
Providing after-care to clients once placed in housing, assisting with transitioning to PSH support services and assisting in navigating barriers to maintaining housing, as they arise;
Initiate case conferences for clients experiencing ongoing barriers to housing;
Work Hours & Position Status
The Outreach Case Manager position is a part-time, 32 hour per week position - Mon-Thursday. Most hours will be during normal business hours (between 8 a.m. – 4 p.m.) but could include some evening, and possible on-call evening hours. There is some flexibility in start-time.
Location
The Outreach Case Manager will work between 3 location sites, within West Palm Beach area.
